Health History

    The following is a list of mandatory documents needed to register a student at Windham Center School:

• Copy of student's birth certificate
• Copy of student's immunization record
• Copy of parent's drivers license showing their Windham address (a must)
• Copy of another form of ID showing their Windham address (piece of mail; purchase and sale of house or a resident form from town hall)
