 2009-2010 Requirements Letter

Dear Parent,   

All school children in the State of New Hampshire are required to have up to date immunizations for school attendance.  For the 2009 – 2010 school year, there have been two changes in the immunizations that are required:  

  1. Tdap:  Required if age 11 or older and it has been 5 years since the last dose of a tetanus toxoid containing vaccine.
  2. Varicella: entering 6th graders shall have received 2 doses of varicella vaccine
  Please provide an updated immunization record to the health office of Windham Middle School before the end of the school year so that we can update our records.  If your child has not yet had the required immunization, please make arrangements soon, and have a copy of their immunization record sent or brought to the health office before the beginning of the next school year.

The NH state law provides that children not in compliance with this requirement will be "conditionally enrolled" or excluded from school.
